Which Membership is For You?

A Full Membership is the membership required for skaters that will participate in fully sanction USFSA competition and will begin/are participating in USFSA's test track (prepreliminary and up).

A Basic Skill Membership is the membership required of all skaters in Basic Skill Levels 3-8 and/or
Freeskate 1-6.
Skaters in Basic Skill Levels 1 & 2 are not required to become members.

Payments for Membership Fees can be made through the club's
Retail Center.
Forms can be mailed and hand delivered to any KMFSC Board Member!

If you are unsure please check with your coach or any board member.

Volunteer Form

Use this form to agree to the volunteer commitment to KMFSC and also choose
what events to work to help meet the volunteer quota.

The volunteer hours are required for Basic Skills Level 4 and above

Group Lesson Registration Form

Use this form to register for learn-to-skate group lessons, the advance jump and spin class on Wednesdays, and/or the intro to jumps and spin class on Fridays. 
If this is the first time you are registering for classes this season remember to include a Membership Form (for Basic Skill Level 3 and up) and Waiver of Liability form (required for all skates). 
Those two forms can be downloaded above!

You can also register for any of the club's group lesson classes online throught the club's
Retail Center.
Remember - registration is due two weeks prior to the first class in each session.


CLASS SIZE IS LIMITED AND STUDENTS ARE TAKEN ON A FIRST COME FIRST SERVE BASIS 

If registration maximums have been met via Park/Rec Depts, YMCA, US Postal Service, Walk-in or Online - you will be notified if a class is full.  
If payment has been received a CREDIT will be issued toward the next session of lessons.
If we are in the last session of lessons a refud will be issued in 5 to 10 days.

Contract Practice Ice Form

Use this form to contract practice ice sessions in advance and guarantee availability of ice for your skater.

If this is the first time you are pre-purchasing practice ice please make sure you include a
Membership Application (for Basic Skills Level 3 and above) Waiver of Liability Form.

Ice Contracts are due one week prior to the start of each new month

Payments are due by the last day of each month for the next month.

Test Session Application

Use this form to register for KMFSC's next test session. 
Test Applications are due fourteen (14) days prior to the date of the test session.  Test Chairs and Coaches can email us their certification/permission to expedite the process!

Apparel Order Form

The apparel committee is in the process of choosing new apparel items for club members.
As soon as that process has been completed the items will be displayed on the website and the order form will be available.

Snowflake Open Basic Skill Competition Forms

Competition information and entry forms will be available as soon as possible.  The competition takes place the first Saturday in December each year and is open to all skaters Basic Skill Levels 1-8, Freeskate 1-6, Prepreliminary and Preliminary!

Professional Coach's Invoice

This form can be used by the professional figure skating coaches to faciliate billing the club for services rendored.
